In order to provide you with the most thorough and personalized care, please fill out the new patient form either by printing out prior to your appointment and bringing completed forms with you to your first appointment, or by arriving at least 30 minutes prior to your appointment time to fill out the required paperwork.

Practice Policies & Procedures

Insurance and Payment Policies:
Patient is responsible for their medical bill and payment is due at the time of service. We do not bill insurance, but will supply you with a receipt to submit to your insurance company. There is a $110.00 no-show and late cancellation fee.
